Protect Yourself From a Lapse in Coverage

Program Eligibility

Patients who have third-party, private insurance, and a current prescription, and are using one of the following types of CSL Behring therapies can enroll in CSL Behring Assurance:

At the time of enrollment and throughout your participation in the program, you must:

  • have third-party, private insurance as your primary coverage
  • be currently prescribed and using an eligible CSL Behring therapy
  • be a US citizen, a legal resident of the US, or have a current US visa*
  • provide documentation of continued product use
  • have met all other requirements, terms, and conditions for eligibility

Third-party, private coverage does not include plans from which funds for payment of claims originate from federal, state, and/or local government budgets, including:

  • Medicare Parts A, B, & D
  • Medicare Advantage Plans
  • Medicaid
  • Medicaid Managed Care
  • Veterans Administration (VA)
  • State Children's Health Plans (SCHIPs)
  • Preexisting Condition Insurance Plans (PCIPs)

* For purposes of this program, eligibility does not extend to US Territories (eg, Puerto Rico, Guam, American Samoa, US Virgin Islands, etc.).

If you have a prescription for Berinert®, C1 Esterase Inhibitor, which might not be used every month, you or your doctor must provide verification that you are receiving ongoing medical care for hereditary angioedema (HAE), regardless of the number of prescription refills in a year, and confirm that you have a current prescription for Berinert in order to receive Assurance points for the prior 12-month enrollment period.

TRICARE and federal or state employees can participate in CSL Behring Assurance provided they are covered by a third-party, private insurance policy(ies).
