Program Points Documentation
To continue receiving your points, you or your doctor must provide documentation annually (once every 12 months, starting from your enrollment date) of your ongoing use of a CSL Behring product.*
You may provide documentation in one of the following ways:
- Ask a CSL Behring Assurance Care Coordinator to contact your physician’s office or pharmacy provider directly to obtain the necessary information.
- Call a CSL Behring Assurance Care Coordinator and provide information over the phone.
- Provide copies of the packing slips from your pharmacy provider showing your name, product brand name, date of shipment, and amount supplied. These can be faxed
or mailed to the CSL Behring Assurance Care Coordination Center.
- Ask your doctor to complete the Brand Certification Form included in the Welcome Packet and return it to the CSL Behring Assurance Care Coordination Center via fax or US Mail.
You will receive a reminder about submitting your documentation approximately 30 days before the 12-month anniversary date.
If the CSL Behring Assurance Care Coordination Center does not receive the information by 30 days before the end of the 12-month enrollment period, your
enrollment in CSL Behring Assurance will be discontinued, thereby invalidating all points you may have earned.
